  1. I am a natural person of legal age, Peruvian or resident, with completed high school studies.
  2. My professional profile fits the following general profile: Innovation managers and technology managers, qualified to intervene and accompany companies in the process of setting up their innovation systems, as well as the structuring and presentation of innovation plans and projects.
  3. I have not been awarded a grant from ProInnóvate (formerly Innóvate Perú) in the following competitions:
    1. Scholarships for virtual courses abroad
    2. Acelera Perú Scholarships, executed by 500 Startups
    3. BAM Grants, executed by 500 Startup
    4. Innovation and Tech Transfer Programme grants, executed by Oxentia
  4. I meet the following legal requirements:
    1. I have a valid ID card or foreigner's card.
    2. I have the document that certifies the completion of secondary education.
    3. I have no criminal or police record.
    4. I have no coercive debts with SUNAT[1].
    5. I am not sanctioned with temporary or definitive disqualification to contract with the Peruvian State, while such sanction lasts.
    6. I have not been convicted by means of a sentence that has become final or has been executed for crimes committed against the Peruvian State.
    7. I am not an employee of ProInnóvate under any modality (276, 728, 1057), nor do I have a consultant contract or service order.
    8. I am not related to persons with employment relationship with ProInnóvate up to the fourth degree of consanguinity and second degree of affinity, by reason of marriage, de facto union or cohabitation.
    9. I do not exercise a public function in Peruvian State Entities.
    10. I am not on the Judiciary's Registry of Unpaid Alimony Debtors.
    11. I am not observed by ProInnóvate or any other funding source to which ProInnóvate has access.
  5. The information described in the application form is truthful.

In addition, I commit to:

  1. Not having another grant awarded by ProInnóvate when starting the course “Companies towards decarbonization”.
  2. Comply with the total development of the course, according to the requirements and guidelines set forth by Libélula. In case I fail to comply with the commitments made, I will be observed by ProInnóvate for one (01) year, during which I will not be able to access the non-refundable resources granted by ProInnóvate, except in cases of force majeure, which must be duly accredited and communicated in a timely manner to Libélula.
  3. Participate and collaborate in the evaluations and dissemination activities developed as part of the course, during the course and upon completion, if required by ProInnóvate.

I also authorize ProInnóvate y Libélula to process my personal data within the framework of Law 29733, Personal Data Protection Law and its Regulation approved by Supreme Decree N° 003-2013-JUS.

Note: ProInnóvate has the power of subsequent inspection according to numeral 1.16, paragraph 1) of Article IV of Law 27444 - General Administrative Procedure Law; it may verify the documentation at any stage of the competition. If falsehoods or inaccuracies are found, ProInnóvate reserves the right to recommend to the pertinent Public Prosecutor the initiation of any legal action.
